Keep your own HTML forms and design while receiving email notifications and reviewing submissions and uploaded files in a customer portal. It supports custom fields, optional acknowledgement emails, your own thank-you page and configurable retention for submissions and attachments. Contact lists are retained separately. Paid plans are available; no free plan or trial is advertised.
